    Getting frustrated with that form.... Sent it by email several times then by fax and they still claim the form was not sent.... I deposited and won $200.00 and they are not sending it to me because of a form... Im going to fax that form 50x a day until they send me the money.... i'll post and let you know how many times it took

    Had absolutely the same problem with them. Won't ever, ever deposit with them again!!! Mine was for $1200!!! Dirty rotten scoundrels!
