Faxback form

Hi Can anyone tell me what this faxback form is? Jackpot Grand is claiming that I cannot withdrawl until they receive it, I'm having a hard time downloading it onto my computer. ThanksJazzyy

 Hi Jazzy:A faxback form is a absolute must for all of us if we plan on withdrawing from a casino.  It is a standard practice, and we all fill them out to verify who we are to the casino.  They ask things like our full name, address, and usually what credit card we used to deposit.  Telephone numbers etc.There is nothing odd about Jackpot Grand asking for one if you have put in for a whithdrawl. Fill it out, and send it back to them as soon as possible, it will only help speed up your withdrawl.Any other questions, let us know.                                                                                                   Pam

I tried and for some reason I cannot seem to download it, im sure its my computer.
